FOXO4-DRI is a synthetic senolytic peptide designed to selectively eliminate senescent cells by targeting a key survival mechanism that allows these cells to resist apoptosis. Cellular senescence is characterized by permanent cell-cycle arrest accompanied by the secretion of pro-inflammatory factors collectively known as the senescence-associated secretory phenotype (SASP). Although senescence plays beneficial roles in wound repair and tumor suppression, the chronic accumulation of senescent cells contributes to tissue dysfunction, inflammation, fibrosis, and a wide range of age-related pathologies. FOXO4-DRI was engineered to disrupt the interaction between FOXO4, a transcription factor enriched in senescent cells, and p53, a central regulator of apoptosis and genomic stability. In senescent cells, FOXO4 binds to p53 in the nucleus, preventing p53 from initiating programmed cell death and thereby prolonging senescent-cell survival.

FOXO4-DRI functions by competitively inhibiting the FOXO4–p53 interaction, freeing p53 from nuclear retention. Once released, p53 can translocate to the cytoplasm and activate pro-apoptotic signaling cascades, ultimately triggering apoptosis specifically in senescent cells while sparing healthy proliferating cells. Preclinical studies in murine models have demonstrated that clearing senescent cells via this mechanism can restore tissue homeostasis, reduce fibrosis, improve organ function, and mitigate symptoms associated with aging, including declines in endocrine function such as reduced testosterone levels and fibrotic changes in various tissues. These findings have generated substantial scientific interest in FOXO4-DRI and related senolytic strategies for their potential applications in regenerative medicine, age-related disease management, and chronic inflammatory and fibrotic disorders. Ongoing research continues to refine its molecular specificity, long-term safety considerations, and translational potential.
